Application To Conduct Credit Investigation and Release Of Credit Information:
The applicant(s) hereby authorize(s) Selling Dealer to initiate a credit investigation based upon the above financial and personal information which information has been voluntarily provided by the applicant(s) and which information is warranted to be true and accurate by the applicant(s). The applicant(s) further warrant(s) that a bankruptcy proceeding is neither presently in progress nor anticipated. The applicant(s) further authorize(s) Selling Dealer to release any and all information obtained pursuant to this application to any individual, business, lender or other institution that Selling Dealer deems to be proper and appropriate.
The applicant(s) authorize(s) Selling Dealer or any lender to whom Selling Dealer provides this information to retain this application and to check and verify applicant(s) credit history.
Ohio law prohibits discrimination with respect to the extension of credit. The law requires that all creditors make credit equally available to all credit-worthy customers and further requires that any credit-reporting agency must maintain a separate credit history on each individual. The Ohio Civil Rights Commission is responsible for the supervision and enforcement of any credit discrimination violations.

The Selling Dealer takes your privacy expectations very seriously. We restrict access to non-public personal information about you to service providers who need to know that information to do mailings and marketing to you. We maintain physical, electronic, and procedural safeguards that comply with federal regulations to guard your non-public personal information. If you learn of inaccurate information, please contact your Selling Dealer to correct it.
